I had a rear brake and stem installed here. On the second ride after installation the bars loosened up as I was coming down a hill and I nearly crashed. The bars rotated down trapping my hands between the brake levers and the bar. I contacted the stem manufacturer who told me that if properly tightened/torqued the bar should have never loosened up.
